擅长心血管病临床诊治和介入治疗,具有丰富临床经验和优秀技术水平。同时,聚焦临床问题,开展冠心病基础、临床、介入手术和预后等方面的系列研究,并取得初步研究成果:
一)动脉粥样硬化“易损性”和冠心病的基础研究
通过系列分子机制研究,阐述内皮功能障碍、SIRT6、TREM1因子及线粒体稳态与斑块易损性的关系。国际上首次发现并报道冠脉心肌桥特征性“FUSIFORM现象”及壁血管“腔小壁薄”的血管生物学特性。腔内影像学技术在体表征心梗患者少见“易损斑块”类型的组织特征(冠脉瘤样扩张、钙化结节病变)。获3项国家自然科学基金面上项目及2项上海市科委课题资助;共发表相关论文15篇 ,其中 SCI 12篇 (总IF 39.5,最高10.1)
二)急性心肌梗死的个体化治疗的基础和临床探索
从心梗的整体化治疗出发,对心梗的“特殊病变、抗血小板药物治疗、紧急再灌注策略、并发症预防和预后改善”等开展研究,提供个体化治疗的证据,包括:国人抗血小板药物的基因多态性与疗效;新型抗血小板改善心梗疗效及机制;应用新型QFR技术优化合并多支病变的急诊再灌注治疗策略;提出心梗术后“补救性”水化方案降低造影剂肾病,以及TREM-1预测心梗事件和死亡风险的危险分层。并尝试探索“斑块糜烂”在心梗患者的发生率、病理、临床特征及转归,筛选和验证早期预警生物标志物,降低剩余心血管风险。获上海市科委重点项目资助(300万);发表相关论文8篇 ,其中 SCI 6 篇(总IF 23.7);部分成果获上海医学会优秀论文二/三等奖。近年来带领团队筛选和验证心梗早期预警生物标志物,完成千余例心梗患者长期随访,同时基于QFR技术,致力探索心梗患者的个体化治疗策略。
三)国产化自主药物洗脱支架的研制和平台构建
在国内最先参与研制中国生物可降解涂层的药物洗脱支架(DES),并最早发表国内研究发现,为填补国内空白做出贡献。从事预防冠状动脉支架再狭窄的基础研究工作,协作建立包括药物支架涂层材料研究、抑制细胞增殖药物的筛选、药物作用机制的阐明、再狭窄动物模型的建立、药物支架疗效及安全性的评价等一套完整的研究体系。申请3项有关自主研制的药物支架的专利,其中2项为第二排名。2001年“国家863计划”资助(重要参与者);2010年“上海市科技奖”一等奖(主要成员);上海心血管病年会优秀论文一等奖(第一作者);主持国家科技部子课题“国产三氧化二砷支架研制和临床应用”
四)优化冠心病介入及精准干预研究
在国内率先引入“支架失败”理念,开展系列冠心病介入技术研究:阐述国人左主干部位的血管生物学特征。国际上创新性提出“弥漫性支架扩张不良”机制是冠脉支架血栓的重要危险因素,并阐明其在支架再狭窄的机制差异,研究结果引发国际心血管同行的极大兴趣,杂志社同期刊文评论和在线视频评论,为改进单纯依赖造影指导介入技术做出重要贡献。在另一方面,阐述DES时代“支架边缘夹层和血肿”发生率、机制及造影不典型表现,为预防严重并发症且避免过多支架植入提供重要理论基础。上述研究结果被他引120余次,其中顶级心血管病杂志JACC、Circulation、European Heart Journal、AJC等40余次;并被写入《血管内超声在冠状动脉疾病中应用的中国专家共识2018》。
综上述,以第一或通讯作者在JACC Cardio intervention, JACC Imaging , JAHA, CCI等发表SCI文章60余篇,中文核心期刊杂志20余篇。出版书籍18本,其中副主编10本、教材1本;副主译1本。共主持国家自然基金项目、上海科委“科技创新行动计划”重点项目等14项,参与国家836、卫生部重点学科课题6项。作为主要完成人获上海市科学技术奖一等奖,中华医学科技奖三等奖,教育部科学技术进步奖一等奖,上海市科学技进步奖一等奖。获得上海市优秀学术带头人、上海市医学领军人才、上海市五一劳动奖章、上海市医务工匠、国之名医等荣誉。
