开创性的使用区域动脉灌注疗法联合床旁血液净化、机械通气及肠内营养治疗急性重症胰腺炎,取得良好疗效; 通过连续心排量监测技术发现慢性阻塞性肺炎急性发作(AECOPD)的呼吸衰竭患者存在不同程度的左心功能不全,监测并改善左心功能,将有利于纠正AECOPD呼吸衰竭。通过临床研究发现,脓毒症患者发生ICU贫血的原因在于红细胞膜磷脂酰丝氨酸外露,引发红细胞衰亡所致。 通过临床研究发现持续静脉-静脉血液滤过对危重症甲型H1NN1流感患者呼吸功能具有保护和改善作用,并用针吸肺活检分析了甲型H1N1流感伴呼吸衰竭患者肺组织的病理损害和特点,据此提出了甲型H1NN1流感呼吸衰竭患者的诊治对策,这对重症甲流、禽流感、新冠等导致的呼吸衰竭诊断与治疗策略的制定提供了理论依据。探讨了脑肠肽Ghrelin在脓毒症中对肠道的保护作用, 证实Ghrelin对脓毒症大鼠小肠上皮短肽载体1具有调节作用,可以改善脓毒症状况下肠道的过度炎性损害并上调脓毒症大鼠小肠上皮短肽载体1的蛋白表达水平,这为改善脓毒症时肠道消化吸收功能提供了有意义的治疗途径. 目前研究方向主要集中在多器官衰竭、肠功能衰竭与肠内肠外营养、脓毒症领域,包括:AECOPD生物节律紊乱的调控及对气道炎性反应的影响机制、脑肠肽Ghrelin对肠道损伤的康复治疗及脓毒症肠道免疫细胞的凋亡的调控治疗、肠道细菌产物易位对肠上皮代谢的损伤、抗生素对肠道微生物平衡的影响等。
发表SCI文章9篇、国家核心期刊论文40余篇;已培养毕业硕士研究生9名;以主要完成人获得省部级科技进步奖二等奖1项,三等奖1项,完成包括国家自然基金、教育部博士点基金、上海市卫健委等科研项目6项,获得国家实用新型专利1项。主要研究领域为急诊危重病、多器官衰竭、肠衰竭。
Continuous infusion of Xuebijing injection through pancreatic local intra—arterial catheter is of better effect than intravenous drug use in treatment of SAP with fewer complication The pathology of patients with type A Hl NI influenza and respiratory failure is similiar with ARDS.Development of treatment strategies targeted to paIhoiogieal characteristics of ARDS caused by type A HI NI influenza is of great significance for effective and timely treatment.For critical patients with influenza A HlNl, CVVH can remove some inflammatory mediators and extravascular lung water,improve respiratory function,and increase the success rate of rescue. The PS exposure of erythrocytes were significantly higher in sepsis patients,and the morphology of red blood ceHs is obvious abnormal.The expression of autophagy associated protein may increase signi6cantly in early stage of small intestinal epithelial cell in sepsis rats,and the decrease f0110wed. The autophagy may be related with bowel dysfunction in sepsis. The colonic PepT1 overexpression induced by bowel resection increases the transport of bacterial product fMLP and hence causes colonic inflammation, which may serve as a previously unrecognized pathway resulting in colon mucosa damage. MDP can be transported through PepT1, causing inflammatory damage in the rat small intestine. Nod2–Rip2–NFjB signaling involved in the small intestinal inflammatory injury caused by MDP which is transported through PepT1. Damage of intestinal bacterial product translocation to intestinal epithelial metabolism.
